Documents & Forms
Enrollment Packet
Registration paperwork for the 2025-2026 school year is available below. Everything you need to complete for enrollment is included. Completed forms may be dropped off at Peace Early Learning Center, in the Peace Lutheran Church office, or emailed to ELCregistration@peacelutheran.net.
All paperwork must be submitted no later than August 1st. Any children with missing paperwork will not be allowed to start until all of the documents are received. This includes:
- Enrollment Application
- Emergency Information And Authorization For Treatment
- General Health Appraisal and Immunization Record completed by child’s pediatrician
- Medication Agreement (if applicable)
- Colorado School Asthma Care Plan (if applicable)
- Colorado Allergy and Anaphylaxis Emergency Care Plan (if applicable)
- School Supplied Sunscreen Form
- Non-Prescription Topical Form
- Social Media Release

Drop-in Care Options
We offer flexible afternoon care for families needing extended hours beyond their child’s regular half-day schedule.
For example, if your child is enrolled until 11:30 a.m. on Mondays, Wednesdays, and Fridays, but you would like them to stay until 12:30 p.m. on a particular Monday, you may request drop-in care by completing a Drop-In Care Form. Please consult the Director for availability and additional details. Drop-in care is offered based on classroom availability and cannot be guaranteed.
Scholarships
Only a certain number of dollars are available and scholarships will be granted per school year based on need, dollars available, and the number of applicants. Please see the director for more information.
Additional Fees
A non-refundable $200 Registration Fee is due with the Enrollment Application. This fee helps to cover the cost of classroom supplies and materials throughout the year. The Registration Fee secures your child’s spot for the 2025-2026 school year. Discounts do not apply to registration fee.
A nap supply fee of $50 will be charged with the first month’s tuition for all children staying after 12:30 pm, which covers the cost of nap supplies and materials.
